There are a number of therapies you could try, although there’s mixed evidence about how effective they are. If you’d like to try any therapies, choose a practitioner who is registered, insured, and has experience of treating pregnant women.

However, colonic irrigation is not an option during pregnancy. Colonic irrigation can affect the normal pH and bacterial balance of your lower colon. There is a risk of complications such as vaginal infections, or more serious problems.

Acupressure or acupuncture

These therapies work on the theory that points in your body can be stimulated to improve your health. An acupressure or acupuncture point related to your digestion is located in the middle of your tummy, about three finger-widths below your navel (point CV6). Constipation is defined as passing hard, dry stool or having fewer than three bowel movements a week. One of the most common digestive complaints in the United States, constipation affects most people at some point in their lives.

Some people experience short-term constipation, caused by temporary changes in diet, travel, stress, or surgery, while others have chronic constipation (persisting for several weeks or longer).

If you have chronic constipation, you likely know how it can significantly affect the quality of your life.

Although constipation can affect anyone, it is more common in women and in people over age 65. It also tends to occur during pregnancy or after childbirth, and it could be the result of an underlying condition or the side effect of medications (such as opioid pain medication).

A number of studies suggest that certain foods and remedies may offer some benefits.

High-Fiber Foods

A diet low in fiber may play a role in constipation. There are two types of fiber: soluble and insoluble fiber.

Insoluble fiber, which passes through the body almost unchanged, gives stools bulk and a soft texture, making them easier to pass. Foods that are high in insoluble fiber includes whole grains, fruit, and vegetables. Try bran, brown rice, or whole-grain bread.

Soluble fiber dissolves in water and forms a gel-like substance in the intestines. Prunes and figs can be added to breakfast or eaten as a snack.

Another option is to sprinkle one teaspoon of ground flaxseeds over any meal. They can be found in packages at the health food store or some grocery stores. They have a mild, nutty taste.

Some of these herbs, such as senna, are approved as over-the-counter treatments for constipation. Although they are meant to be short-term treatments, in reality, people may become dependent on them and use them for weeks, months, or even years at a time in order to have a regular bowel movement.

Biofeedback therapy may help people with constipation resulting from pelvic floor dysfunction, a condition in which the pelvic floor muscles do not function properly. It occurs as a result of conditions such as obesity, an enlarged prostate, or after childbirth.

Biofeedback therapists teach how to better coordinate muscles used for defecation (anorectum and pelvic floor muscles). Although biofeedback has only been explored as a treatment for this type of constipation relatively recently, results are promising.

If you're taking an iron supplement for anaemia, you may find that your constipation gets worse, as this can be a side-effect of taking the tablets (Bianco 2017, NHS 2016a).

If you don’t exercisemuch, your whole system will be sluggish. As your pregnancy progresses, and your baby gets bigger, the pressure he puts on your pelvic area makes constipation more likely.

If you have severe constipation and have to strain to poo, it increases your risk of developing piles(haemorrhoids). Continuing to be constipated can make piles worse, so the sooner you tackle constipation, the better.

How can I prevent constipation?

Eat a high-fibre dietthat contains lots of wholegrain cereals, wholemeal bread and fruit and vegetables. This will provide you with roughage to aid your digestion. You should also drink at least 1.5 litres (2.6 pints) of water every day.

Don’t rely on taking bran to avoid constipation, unless you also drink a lot more fluids. Without extra water, bran will bulk up your poo, making it hard and less able to pass along your intestines. Large amounts of bran may also interfere with your body’s ability to absorb certain nutrientsfrom your food.

Staying active and exercising will help to prevent constipation. Walking, swimmingor aquanatal classes, gentle cycling on an exercise bike, or pregnancy exercise classes can all help.

Cut out drinks that make you wee more often, such as tea, coffee, and cola. These diuretics may dehydrate you and make your constipation worse.

What tips can I try to ease constipation?

Start your day with a cup of warm water and a slice of lemon before you have anything else to eat or drink.

Start each meal with salad or raw vegetables or fruitand try to have some of the following every day:

  • Fresh fruit, such as oranges, grapefruits, tangerines and grapes.
  • Dried fruit, particularly prunes (Attaluri et al 2011).
  • Vegetables, such as celery, watercress, cabbage, and spinach.
  • Wholegrain cereals and wholemeal bread.
  • Beans, lentils and pulses.

Psyllium (ispaghula husks), the seeds from plantain, may be effective for constipation (NICE 2015), especially if you have irritable bowel syndrome (IBS). Psyllium contains mucilage, which bulks up your poo.

You can buy psyllium in health food stores, but check with your doctor or midwife before taking it, especially if you are taking medication (Stewart 2016). Psyllium can interact with some medicines.

Linseeds (flaxseeds) are rich in fibre and essential fatty acids, and are another alternative to psyllium. Try sprinkling a few seeds on your breakfast cereal, salads, or yoghurt. Drink at least one large glass of water for each dessertspoonful of linseeds you have. Too many linseeds may cause diarrhoea and, very rarely, can trigger allergic reactions (Álvarez-Perea et al 2013).

Supplements

Iron supplements to help with anaemiacan make you constipated. Ask your midwife or doctor to prescribe a different brand of iron tablet if the one you are taking makes you constipated. A liquid iron supplementmay suit you better.

If you’ve been diagnosed with anaemia, try to eat red meat or pulses, and dark green vegetables in your meals, as well as taking your prescribed supplement (Pavord et al 2012).

It’s possible that probiotics may help to relieve pregnancy constipation (De Milliano et al 2012). It’s best to talk to your midwife or doctor about which supplements to try.

If constipation becomes a more serious problem, your doctor or midwife may prescribe a laxative that’s safe to use in pregnancy.

Pelvic floor relaxation

When you feel the need to go to the toilet, try to give yourself privacy and time. Sit on the toilet, breathe deeply, then exhale so your pelvic floormuscles relax, but try not to strain.

While sitting, go onto your tiptoes to lift your knees above your hips. Or, use a footstool to raise up your feet. This will put you into a squatting position, which is the ideal position to do a poo (NHS 2015a).

Which complementary therapies could I try?

There are a number of therapies you could try, although there’s mixed evidence about how effective they are. If you’d like to try any therapies, choose a practitioner who is registered, insured, and has experience of treating pregnant women.

However, colonic irrigation is not an option during pregnancy. Colonic irrigation can affect the normal pH and bacterial balance of your lower colon. There is a risk of complications such as vaginal infections, or more serious problems.

Acupressure or acupuncture

These therapies work on the theory that points in your body can be stimulated to improve your health. An acupressure or acupuncture point related to your digestion is located in the middle of your tummy, about three finger-widths below your navel (point CV6). Gently press it intermittently, about 20 times to 30 times, and repeat several times a day.

If your constipation persists, you could consult a qualified practitioner of acupuncture or acupressure. There is some evidence that acupuncture relieves the symptoms of irritable bowel syndrome (IBS) (NHS 2016b). Constipation is a symptom of IBS, but we can’t say for sure that acupuncture can ease constipation during pregnancy.

Aromatherapy

Add three drops of essential oilsuch as sweet orange(Mantle and Tiran 2009), lemon, lime, grapefruit or bergamot to 5ml of carrier oil (preferably grapeseed) to your bath. Relax in the warm water for a while, perhaps stroking your tummy in a clockwise direction.

Don’t press your belly too firmly, especially if you are in your first trimester, have any risk of premature labour, or if your placenta is lying low in your womb(placenta praevia).

Herbal remedies

You could drink dandelion or mallow tea, made from the leaves of the plants. Steep the leaves in boiling water and drink it daily to ease constipation.

Senna is a traditional remedy for constipation and is safe in small doses and for short periods in pregnancy under the supervision of your doctor or midwife (NICE 2015).

However, don’t take senna in your third trimester (NHS 2015a), as it may stimulate your womb (uterus) and cause powerful Braxton Hickscontractions.

Homeopathy

There’s no evidence that homeopathy is effective (NHS 2015b). However, if you want to try it, make sure that you see a qualified, registered homeopath.

Osteopathy

Osteopathy or chiropractic may correct any misalignments in your skeleton. In theory, a previous injury could be putting stress and tension on soft parts of your body, including your stomach and intestines.

There’s evidence that osteopathy can help with mechanical problems, such as lower back pain, and possibly shoulder and neck pain (NHS 2015c). But we can’t be sure whether this could affect how your digestion works during pregnancy.

Reflexology

Reflexology is based on the idea that your feet correspond to a map of your body.

You can try a simple reflexology treatment at home. Ask your partner or a friend to massage the arches of your feet. Reflexologists say that the arches correspond to the digestive system. Your partner should massage in a circular, clockwise motion, as this is the same direction as the movement of the intestines. Ask your partner to spend about five minutes on each foot.

If you would like to massage yourself, you could try using the reflexology zones on your hands. Massage the dip in the palms of both hands in a clockwise movement. Or put two bottles under the arches of your feet while you’re sitting, and gently roll them backwards and forwards on the floor.

If you want to book a session with a reflexologist, you’ll have to pay for it. Just bear in mind that there’s no evidence that reflexology helps with medical conditions (Ernst 2009).

Constipation in children

Early identification of constipation and effective treatment can improve outcomes for children. Without early diagnosis and treatment, an acute episode of constipation can lead to anal fissure and become chronic.

The first-line treatment for children with constipation requires the use of a laxative in combination with dietary modification or with behavioural interventions. Diet modification alone is not recommended as first-line treatment.

In children an increase in dietary fibre, adequate fluid intake, and exercise is advised. Diet should be balanced and contain fruits, and vegetables, high-fibre bread, baked beans, and wholegrain breakfast cereals. Unprocessed bran (which may cause bloating and flatulence and reduces the absorption of micronutrients) is not recommended.

If faecal impaction is not present (or has been treated), the child should be treated promptly with a laxative. A macrogol (such as macrogol 3350 with potassium chloride, sodium bicarbonate and sodium chloride) is preferred as first-line management. If the response is inadequate, add a stimulant laxative or change to a stimulant laxative if the first-line therapy is not tolerated. If stools remain hard, lactulose or another laxative with softening effects, such as docusate sodium can be added.

In children with chronic constipation, laxatives should be continued for several weeks after a regular pattern of bowel movements or toilet training is established. The dose of laxatives should then be tapered gradually, over a period of months, according to response. Some children may require laxative therapy for several years.

A shorter duration of laxative treatment may be possible in some children with a short history of constipation.

Laxatives should be administered at a time that produces an effect that is likely to fit in with the child's toilet routine.

Faecal impaction in children

Treatment of faecal impaction may initially increase symptoms of soiling and abdominal pain. In children over 1 year of age with faecal impaction, an oral preparation containing a macrogol (such as macrogol 3350 with potassium chloride, sodium bicarbonate and sodium chloride) is used to clear faecal mass and to establish and maintain soft well-formed stools, using an escalating dose regimen depending on symptoms and response. If disimpaction does not occur after 2 weeks, a stimulant laxative can be added or if stools are hard, used in combination with an osmotic laxative such as lactulose. Long-term regular use of laxatives is essential to maintain well-formed stools and prevent recurrence of faecal impaction; intermittent use may provoke relapses.

Hemorrhoids — swollen veins in the anus and rectum — are common during pregnancy, especially in the third trimester when the enlarged uterus puts pressure on the veins.

Hemorrhoids can be painful. They may also itch, sting, or bleed, especially during or after a bowel movement.

While your body is going through all sorts of physical changes during pregnancy, hemorrhoids can be one more unwanted irritation. But the good news is that they generally aren't harmful to your health or the health of your baby, and they’re usually a short-term problem. Though pushing during labor can worsen hemorrhoids, they typically go away on their own after you give birth. (1)

Some women get hemorrhoids for the first time when they’re pregnant. But if you’ve had hemorrhoids before, you’re more likely to get them again when you’re pregnant.

What Can Cause Hemorrhoids During Pregnancy

As your unborn baby grows, your uterus gets bigger and begins to press against your pelvis. This growth puts a lot of pressure on the veins near your anus and rectum, and these veins may become swollen and painful as a result.

The increase in the hormone progesterone during pregnancy can also contribute to the development of hemorrhoids, as it relaxes the walls of your veins, making them more prone to swelling. An increase in blood volume, which enlarges veins, can also contribute to hemorrhoids during pregnancy. (1)

Three common additional causes of hemorrhoids during pregnancy include:

  • Straining during bowel movements
  • Straining from carrying extra pregnancy weight
  • Sitting or standing for long periods of time

Hemorrhoids are most common in pregnant women who experience constipation.

As many as 38 percent of pregnant women become constipated at some point during their pregnancy, according to research published in the journal BMJ Clinical Evidence. (2)

One cause of constipation during pregnancy may be when the growing uterus pushes against the bowel. Iron supplements you may take can also contribute to constipation, so it’s worth trying to get the iron you need naturally through your diet. (3)

Pregnancy hormones can also slow down the movement of food through the digestive tract, making constipation more likely.

How to Prevent Hemorrhoids During Pregnancy

Avoiding constipation is key to preventing hemorrhoids during pregnancy. Here are some tips for preventing constipation:

Eat lots of high-fiber foods. There are plenty of good ways to incorporate more fiber into your diet. Fiber-filled foods include fruits like pears (especially when you include the skin), avocados, and berries; vegetables such as broccoli, artichokes, and Brussels sprouts; whole grains such as oatmeal, brown rice, quinoa, and even popcorn; legumes including various kinds of beans, lentils, and green peas; and don’t forget nuts and seeds.

Drink plenty of fluids. Aim for 10 8-ounce glasses of water each day.

Use the toilet as soon as you feel the urge. “Holding it in” can contribute to constipation.

Try not to sit or stand for long periods of time. If you sit down at work, make sure to get up and walk around for a few minutes every hour. At home, try to rest on your side when reading or watching TV, to relieve downward pressure on your rectal veins.

Ask your doctor about using a stool softener. This can help if other methods fail to ease your constipation. Using laxative pills for constipation is not recommended during pregnancy, as they can cause dehydration and might stimulate uterine contractions. (3)

Do Kegel exercises daily. Kegel exercises strengthen the pelvic floor muscles that help support your rectum and can improve circulation in the rectal area. You can do Kegel exercises just about anywhere — at home, in your car, at the office — but first you need to make sure you’re isolating and contracting the correct muscles.

Identify the right muscles by stopping urination midstream. (This is strictly for the purpose of identifying which muscles we’re talking about — you don’t want to do Kegels while urinating, as this could increase your risk for a urinary tract infection). (4)

Once you know which muscles to use, tighten them and hold the contraction for five seconds. Then relax for five seconds. Work up to holding the contraction for 10 seconds. Try to do at least three sets of 10 reps a day.

How to Treat Hemorrhoids During Pregnancy

Hemorrhoids usually get better on their own after pregnancy, but there are a number of things you can do to ease any itching and pain in the meantime:

Soak your rectal area in warm water several times a day. A sitz bath, or small basin that fits over the toilet seat, can help. These devices can be purchased at most drug stores. You can also fill your regular bathtub with a few inches of warm water to create a similar effect.

Apply ice packs or cold compresses to the area several times a day. The cold can reduce swelling and help relieve pain.

Keep the anus clean and dry. Try using moist towelettes or baby wipes to gently clean the area after bowel movements. This can be more gentle than dry toilet paper.

Be sure to pat — not wipe — the area dry after bathing or making a bowel movement. Excess moisture can cause irritation.

Apply baking soda (wet or dry) to the area to alleviate itching. (3)

Apply treatments containing witch hazel. Products like Tucks Medicated Cooling Pads can help keep the area clean and relieve pain and itching around the anus.

But before you use any products, be sure to ask your doctor or healthcare provider to recommend a topical hemorrhoid cream or medicated wipe that's safe to take during your pregnancy.
